Can someone help with a third appeal letter

My insurance company denied me for the 2nd time. I was denied because my previous PCP would not provide the insurance company with a complete medical history of the diet attempts. The appeal nurse told me to wait until February to file a new claim. I was given 60 days to appeal the 2nd denial.
