Desperate for medicaid with doctor approval in Florida

I was just let go from my job and no longer have my cigna insurance. I was let go because of great depression due to being obese and also being ill from my weight. Because I am a single mother I can not afford to pay out of pocket for this surgery. I am desperatly seeking any help I can get for info on a surgeon that accepts medicaid in florida or a state that is close to Northwest FL. I am desperate I was already approved and waiting for a surgery date.
