Need a TT and hernia repair...anyone dealt w/self insured comapny?

Need a TT and hernia repair...anyone dealt w/self insured comapny? We use Cigna HMO but we are a self insured company. I am hoping that someone can give my some info on that journey. Paper work has been sent in to to insurance company. PCP wrote letter as well since he has treated fungal infections. When I went to visit the plastic surgeon he also spotted another infection that was healing. Please help me. I need to know what to do if I get denied.
