Does Aetna cover WLS no matter what company you belong to, or do some plans exclude?

I was wondering does Aetna general cover WLS regardless of the company or organization you get it through, or do some companies with Aetna have an exlusion clause for weightloss? Generally speaking, does aetna always cover WLS if you meet criteria?
