I need a PCP in the Dallas, TX area on Aetna HMO that will refer me

Does anyone know a PCP in the Dallas, TX area on Aetna HMO that will refer me to a surgeon and even write a recommendation letter. Will be willing to try a diet. I dont have any co morbs that I know about. My BMI is 41.9.
