If Insurance denies surgery coverage, who writes the appeal?

It seems like this is taking FOREVER! I'manticipating a denial since my insurance keeps telling me how this is considered "cosmetic". I am wondering...If I am denied, who will need to write the letter of appeal? Me or the doctor? If it's me, I want to get started and have it ready. Please answer....
