Decided to have the surgery and pretty sure insurance will pay, what next?

I have called a bariatric center and the lady there told me that they had no openings until August 1st. She also mentioned something about sending me out information and papers for me to fill out, a week later I still haven't gotten anything. Any advice? What should I be doing to get the ball rolling?
