How does one get an insurance company to listen to what a DOCTOR says?

Both my primary care physician and the surgeon he referred me to for RNY surgery agree that I am a good candidate for the surgery and that it is medically necessary. HOWEVER, some *&#%* at the insurance company overruled their determination and said that it's NOT medically necessary. How is this possible? I am in the process of appealing the decision but I am feeling like it may be a lost cause. Does anyone have ANY help they could give me, suggestions, what they did, etc. that I could pass on to my doctors to help get the surgery approved? I would REALLY appreciate any help I can get. Thank you !
