Anyone know PCP in San Bernardino, CA w/Pacificare for referral?

I need a PCP in San Bernardino/Redlands area who is BLS friendly. My PCP refuses to send anyone to this kind of surgery. I easily qualify. I have Pacificare at Beaver Medical Group in Highland, CA.
