I have just received a letter from JF Molloy insurance denying surgery.

The reason is because our employer has an exclusion that states surgery or weight loss programs due to obesity are not included in the plan regardless of metabolic or endocrine orders. Has anyone else had an exclusion like this & appealed it & won?
