I'm having my first appt. with my Doctor to request a referral for WLS

What can I say to him to convince him that this is the right thing for me and to give me the referral to a Surgeon. This is a new Primary Care Doc and I don't know how he is aobut the WLS. My insurance ins Prudential HMO and they said they will ok it if he says it's medically necessary. I have HBP, degenerative arthiritis, and weigh about 345 at 5'5. I'm 38 years old with a 3 year old son that I want to play with and see grow up. I just have this terrible feeling that if I don't do this surgery i won't live too much longer. Any suggestions with what to use in my defense? Thanks, Shelli
