Should I see my pcp first or got on to the surgeon.........

I am from Indiana and have Mplan as my insurance. Has anyone been approved and if you have how did you go about it? I don't think my pcp will refer me so should I go on to the surgeon and let them handle it? I am 25yrs., 350lbs, 5'5", and am really considering surgery. Thanks!!!
