Is the lack of a menstrual cycle, headaches, & back pain enough to justify surgery?

The policy of my insurer is to pay for WLS given "serious obesity related medical conditions" just wanted to see if these would fall into that category. I'm fortunate enough to not have joint pain, diabetes, or hypetension...YET.
