sheduled for first consult

got a call from dr. thoburn in Gainesville, Fl. They seem to think that i would be a good candidate for wls... My BMI is 45.1... I am scheduled for my first consultation on December 10th... any advise on what to say or take with me would be appreciated... also if anyone has used him how is he?
