Aetnas CPB 157- Multiple Disciplinary Program

I am wondering if anyone has been approved by following the 3 months of requirements that Aetna now offers? My surgeons office has never had anyone approved so I want to make sure I have all my ducks in a row when I submit for insurance approval next week.
