I became enrolled on husband's policy in Jan. 2005. When surgeon's office verified my coverage for WLS, BC/BS said that the policy has a rider, which only allows people enrolled on policy before a certain date to be eligible for obesity surgery. I was not enrolled before the date, therefor the service is not a covered benefit. Can this be appealed or is it set in stone?
