need a breast reduction and don't know where to start.

I've lost 90lbs and I'm 11 months post op. My breast hurt so bad from my weight lose.I was in pain before weight lose but I'm really in pain now.But I don't know how to get started. Do I consult my pcp or the surgeon that I want to do the surgery. will someone please tell me where to start
