My PCP told me she would write a letter for me to submit to my insurance comp

My PCP told me she would write a letter for me to submit to my insurance company to help me get approved for my tt, thigh lift and lipo. She said I should write down what I want her to include in the letter. Does anyone have any suggestions on what I should have her write? Any help would be great! Thanks so much!
