I need some help for insurance approval for TT

I am trying to get approved for a TT. I have never had any rashes at all, and that is one of things that I have to have to get the surgery... it has to be medical necessary! No I dont have a herina, rashes, or any other document problems........any suggestions??? Also time is running out, so I need some help quick!
