Anyone know what humana hmo requires before surgery ?

I have humana hmo , i go once again for a weigh in on monday . i had ppo this past policy and hit a road block with the exclusion towards surgery . so i chg'd over to hmo . now I am wondering if there is going to be any surprises when i go to get weighed , as to what the ins. will require . i meet their must be morbid obese and med. necessary , and they told me afew months ago their requirements , but i want to make sure . also i am in texas , sometimes that makes a difference .... thanks
