Ins requiring history of medically supervised wieght management???

My insurance just asked for documentation of a medically supervised weight management intervention that would include medical nturition therapy, behavioral interventions, suprevised increase in activity with relapse prevention/maintenance??? My bariatric office suggests I get a letter from my primary but I have never had medically supervised diets - I've lost 100 lbs twice on my own through diet and exercise and gained all back twice plus more. Anyway had this experience and had it approved???? Please give me your experiences and letter that did the trick???? Thanks so much!!! Frustrated - want to begin new life now that I am settled on surgery - don't want these delays....
