Seeking those with United Health Care and had surgery in the Jacksonville, FL area?

I am having lots of trouble finding a doctor in my area. The insurance company does not have a listing of bariatric surgeons and have told me to find one by calling the general surgeons. This is not working. Out of the 4 listed, 1 is out of network, 2 not on the insurance at all and the 1 that is on the insurance want $6565.00 upfront. (the ins com pays 100%) go figure. Any help is greatly appreciated and needed. Thanks
