can someone tell me what united health care of florida looks for thier approval?

If I dont have surgery before 8/1/03 our insurance is most likely changing to UHC I would like to know what they look for specifically for approval I know it has to be medically neccesary but whaT EXACTLY WOULD I need? Thank you for your help..
