HELP Pacificare Promed IPA, So Cal.

Help again please, My mother called her insurance and found out Pro-med IPA is part of Pacificare!?????? What does this mean? They wouldn't give her any info about what they require for gastric bypass surgery! All they have told her is that is up to her surgeon. Please help! She is going to go to a medical group in Pomona, CA. She wants to knwo if they require a 6 month supervised diet? Do they require pysch eval?
