As of 5/16/03 criteria w/bcbs fl changed and you need 5 yrs.of being obese but?

How do you go about prooving this they already have 2 years from me and proof from 1999 from me so I guess I will have to get 2000 and 2001 since they already have 2002 and 2003 The criteria changed as I was in middle of an appeal with them but I need to know can the proof be just from 1 visit in that whole year with a doctor or emergency room that has proof of my weight on it all I have is 1 visit in 2000 w/ planned parenthood that has my weight and in 2001 I have a emergency room visit with my weight on it is that enough to satisify insurance? Also do they look at it 5 years being 1999,2000,2001,2002 and 2003 or does it have to be 1998-2003? thank you for any help...
