Pacificare HMO Approval?

I have Pacificare HMO through Washington State Employees. I was wondering if anyone has ever gotten past the written exclusion and if so how? I have been denied once, and my doctor sent a second appeal for me. Was wondering if and how to get approved with these guys. Thanks in advance.
