Does BCBS TX ever approve Lap Band

OK I know most of you are not big on lap band, however if there are any of you out there who have been banded AND were approved by BCBS TX PPO would like to hear from you regarding which surgeon you used, how recently you have had your surgery,BMI and comorbidities.Wondering if they cover this procedure at all.
