Would like info from pts of Dr Harberg who aso have BCBS TX PPO

What information had to be submitted to the insurance company and was required by Dr.to be approved? Was a psych evaluation, meeting with a nutritionist and letter from PCP required? Also I had a physical last August with my PCP with extensive lab work should I bring this or just let Dr. Harberg repeat it? My initial consult is in 4 weeks YAHOO!
