How can i get my old pcp to help me.

I have been through many many "hoops", and actually have surgery scheduled for the 7th of January, however now the insurance co. wants the documented proof of non surgical weight loss attempts for the past 3 years. I have none. Mostly because my old pcp was .. shall we just say less than supportive.... anyone have an idea on how to get him to help me with this LAST hurdle?
