Anyone have a source of a checklist to facilitate approval by ins. co.?
I've read so many stories of approval and/ or surgery dragging out over many months. Is there an efficient way to approach this, preferably a checklist detailing the steps to take, so that delays can be avoided? Are there "boilerplate" letters used for crafting requests and appeals? If so, source, please?
