Does anyone have a letter of medically necessary from their doctor to the insur. comp

WLS is not covered on the policy I have with BC/BS of South Carolina unless it is medically necessary. My family doctor has referred me to a surgeon but the insurance company wants a letter of medically necessary. Does anyone have a letter that my doctor could use as a guide line? If so could you please email it to me ASAP. Thanks
