My insurance (health assurance ppo ) want me to provide a letter of past diets

I went to a medically supervised diet center. That was Feb.01. Lost some weight and stop May 01. Gained back all the weight and more. My insurance want recent weight loss attemps within the last 6 month. My PCP is very strick and does not promote anything , but counting calories. What should I do. I could go back to the weight loss center, but well this be enough. My appointment well be in June with the doctor I want the surgery with. Well my insurance accept a letter from a weight loss center with a doctor.
