Has anybody had problems being approved after submitting weight and diet history

I have BCBS and they want weight and diet history. Has anybody had a problem with approval after submitting. My diet history is diet pills and slim fast. I havn't taken the diet pills in the past 7 months because of the way they make me feel. But have done the slim fast. The info doesn't have to be medically supervised.
