insurance denied

I received a letter from insurance this morning and was denied.(bc/bs ppo) All it said was that I was denied. I won't be able to find out until Monday why. First I was heartbroken but now I am mad. Need help on a good appeal letter. All help appreciated. E-mail at [email protected] thanks rojo
