Has anyone recvd approval from Aetna HMO without prior documented diet history?

I just got a call from my surgeon's office, and they said that the nurse at Aetna says I need documented history of Medically supervised diets. My PCP up to age 22 (i'm 28 now) has deleted all of my records.. PLEASE SOMEONE TELL ME A SUCCESS STORY.
