Need well-worded appeal letter re: not enough

My PCP is willing to write whatever I tell him regarding denial due to not enough medically supervised diets. Would like to see letters others have sent to insurance regarding this same subject...did anyone else write the letter for their PCP? Please e-mail to me at [email protected].
