Can someone PLEASE help me--Pacificare HMO denied because its a revision.

Has anyone out there dealt with this issue. I have my original surgery in 1982 and it has failed. I presently weigh 260# (5'2") with com-morbitities of osteoarthritis, shortness of breath, depression(which has been treated with medication for several yrs.)I NEED HELP TO DO MY APPEAL.Thank you.
