I have smoked for 21 years and am having a real tough time kicking the habit

I am low-income, and am rolling my own ciagretts. I want to try the patch, but cannot afford them. It is cheeper to smoke rollies. Is there anyone willing to help? I am afraid that if I don't do something fast, my PCP will not allow me to go through surgery. He told me flat out that he would not allow me to do this as a smoker. I tried and tried with no luck. I have been waiting a long time, and I just finished my last appointment before approval. Somebody, please, HELP. Thank You for listening, and if you can help, please e-mail me from my profile.
