Does the patch really work?

I have been a smoker for 21 years, and am having a tough time quitting. Does the patch really work, and how soon should I start using it? I can hardly afford cigaretts, so I roll my own, and the patch is not covered on my medical. Is there anyone out there that knows a way I can get help obtaining the patch. I am afraid that if I don't do something now, my PCP will not allow me to go through it. Roll your own cigaretts are much cheeper than the patch. Help. By the way, I would like to thank everyone who responded to my question about needing help with clothing.
