My 1st attempt at RNY surgery by my PCP was denied I'm on disability what next?

I Have been on Social Security Disability for a back injury for 10 years.For the past 2 years I've purchased coverage thru Humana Gold Plus (an HMO) of KY. I am 400lbs and want RNY surgery. I am told by my Dr that I was denied coverage, which is Humana policy; deny all first requests for surgery that are non life threatening. What would be your suggestions as to my next move? Doctor wrote a letter of medical necesity with no luck.I have read my plan exclusions and gastroenterostomy is not mentioned as an exclusion. Your help is greatly appreciated.
