I am looking for a supportive PCP in the no. cal area for surgery referral?

My PCP is non supportive of my choice to seek the gastric bypass surgery. My BMI is 42 and I suffer from back pains and high blood pressure. She continues to refill my presc. for xenical but want even consider me as a surgery candidate. I feel hopeless, if anyone has any suggestions or referrals please help a desperate soul. I am located in the Bay Area.
