Need Advice on these Lengthy Processes...

I have Humano HMO. Can anyone provide a sample letter from PCP's for medical necessity? My PCP is working on it, but I want him to have all that he needs to have for insurance purposes. Also, can anyone share their process on getting the nutrition and psych evaluations? Blessings!
