Has anyone had UnitedHealthCare in Orlando pay for TT/Hernia repair?

I have a way to go but I'd like to know what kind of documentation do I need to start compiling for when I reach my goal weight. I already have an orthopedic for my osteoarthritis,dermatologist for skin infections and hve seen a surgeon that says it's not worth repairing my hernia until I lose the weight. What do I need to have United in Orlando cover a pan/hernia repair. Does one have to be at goal weight and for a specific time?Should I continue to go to Drs. only to here you need to lose weight in your abdominal area? Thanks
