Please help, I can't find a letter I read here once, and I need it!!!!

A little while back a found a sample letter that someone had posted that was from their psyc evaluation. I am having surgery on Dec. 9th. and need one. I have someone to do it for me, but he does not do them on a regular basis and ask me to show him the sample that I mentioned. Of course, I can't find it now, PLEASE HELP!!!!
