been denied by Aetna HMO

I just found out today that Aetna HMO has denied my surgery scheduled for Oct. 23rd because I haven't been morbidly obese for five years. I would appreciate any suggests. I was thinking of trying to pay cash, and in the mean time file an appeal. Has anyone done this and been renburst for the surgery?
