What does Dr. Todd McCarty require before making an appt?

After reading and researching surgery I've decided I want to start the process. I like what I've read about Dr McCarty and he is on my insurance list. I'm just not sure where to start. I read in someone's comments you need to attend a seminar and have approval first. What else does he need? I plan to make an appt with my PCP but was curious to be more informed before I go. Thanks!
