I'm applying for an individual policy and I'm not sure how to answer...

The following 2 questions are asked on the application for Anthem BC/BS plan: Has any person applying for coverage been hospitalized, had surgery, or advised that hospitalization or surgery may be required in the future for any reason, including inpatient and/or outpatient surgery? Has any person applying for coverage sought advice or medical treatment, or been advised by a medical or social practitioner to seek advice or treatment for any condition or symptoms not indicated by your answers to any questions in the preceding section(s)? I was declined by my current policy b/c of an iron-clad exclusion (United Healthcare) about 6 months ago. Do I have to answer yes to these questions?
