Has anybody been approved by First Health PPO through State Farm Group Insurance plan

I have been through the Insurer research info on this site but can not find any info here in PA. I have a surgery date 3/20/02 and really need to know if they approve with minimal co-morbs but have a BMI 50(+/-). Thanks for any input. Happy Holidays! Debbie
