Does anyone have Blue Cross/Blue Shield Alliance in Missouri?

My husband is considering taking a new job and they have BC/BS Alliance...now I am not sure if it is ppo/hmo, but they did state there were co-pays. So I am assuming it is PPO/HMO. Does anyone know if they cover the surgery usually and what about Pre-Exisiting? Thanks in advance for all your help!!! Marie
