I submitted family history and self-evaluation. Did I ruin my chances?

I am looking at these posted questions about Psych evaluations and most answers are saying don't admit too much. Oh god I think I may have ruined my chances. I am 22 and admitted to having a 3 month affair with methamphetamines (one of my so-called diets)when I was 16, I also admitted to having been on 3 different anti-deppressents. and too have been to therapy twice (once for a year and another for 4 months. Now I am fine and on no meds, but before i was so depressed and with lo-self esteem. Does this ruin my chances. I completely understand the surgery and risks and have contemplated it a year before I actually acted decided to go for it. Now, I'm scared I have ruined everything...any advice?
